Interested applicants must submit the following items as one PDF file to Stephanie Watson at Current CV, medical school diploma, medical school transcript, MSPE (Dean's letter), 3 letters of recommendation (current/past program director required), USMLE transcript Step 1, 2, CS and 3, personal statement, current ACLS certificate, ABSITE transcript(s), milestone report(s) to date, ACGME defined category operative log, list of completed rotations, and a competency-based summative evaluation for PGY-1 year of training.
